Focal have been providing services for 49 years in the Ipswich, Brisbane and Lockyer regions of Queensland.Focal are looking for people with industry knowledge, who are seeking a change in career to apply for a Support Coordinator position and join our team.As a Support Coordinator, you will: • Work creatively and resourcefully with NDIS participants in how they utilise the support budget in their NDIS plan to achieve their goals• Assist the participant to link to mainstream or community services (i.e., housing, education, transport, health) where applicable• Build the capacity of the participant to exercise greater choice and control and develop skills for increased independence in directing services and supports in the longer term• Attending home and community visits to connect, build relationships and capacity, and ensure that the quality and value of the supports received, meets expectations.• Ensuring all the necessary "paperwork" including case file notes and reporting is completed in a timely manner, to meet NDIS and sector requirements and guidelines.• Advocating for individuals and their families with dignity, communicating their goals and aspirations to their stakeholder team, and the National Disability Insurance Agency.About the RoleThe National Disability Insurance Scheme (NDIS) supports people living with a broad range of physical, intellectual, and psychosocial impairments by providing packages of assistance. These packages outline a plan and budget that is able to be used, to access a range of supports a person needs. At times, understanding how this budget is utilised and allocated can be tricky, and this is where a SupportCoordinator comes in!A Support Coordinator works directly with people receiving NDIS funding and their families, to remove any barriers in navigating their package and establishing supports. This assistance aims to build capacity and confidence around coordinating and managing their own supports, over time.Essential Requirements:Our ideal Support Coordinator will have well developed interpersonal and communication skills, with the ability to network with local service providers. You will also demonstrate sound organisation and have a high level of attention-to-detail, ideally you will have:Certificate IV in Disability / Mental Health or other higher qualifications in human servicesPrevious experience working with people with a disability or mental health diagnosisDemonstrated knowledge and experience with meeting legislative requirements including strict adherence to privacy and confidentialityExperience designing and managing client plans independentlyKeep abreast of current NDIS legislation, rules and operational guidelines (including NDIS Quality and Safeguarding Commission, NDIA and other relevant peak bodies)Demonstrated organisational and time management skillsGoal driven and consistently meets deadlines and key performance indicatorsCurrent NDIS Worker Screening Check or willingness to obtain oneCurrent Paid Blue Card or willingness to obtain oneFully vaccinated against COVID-19Current National Police Certificate (issued within the last 90 days)Current driver’s license and access to a reliable, registered, comprehensively insured vehicle Queensland has introduced a "NO CARD, NO START" policy.  Please make sure you understand the requirements for a Disability Worker Screening Clearance.
